If you are the parent/guardian of a student who is new to both the Jordan School District and Sunset Ridge Middle School, you will need to do the following:
- Come to Sunset Ridge during normal business hours and bring the following:
- Proof of Residency: This is ONE (1) copy of your mortgage or lease agreement (parent/guardian's name MUST appear on the document) and TWO (2) additional proofs (such as utility or other bills)
- Bring a copy of your student's current immunizations (current required immunizations are listed here)
- Bring an original copy of your student's birth certificate
- A parents driver's license.
